Mental Health Support for Chronic Illness in Manhattan, NYC
Living with a chronic illness changes everything: your body, your sense of self, your relationships, your plans for the future, and your relationship with hope. The psychological weight of that change, the grief, the depression, the anxiety, the identity loss, is real, significant, and consistently undertreated. At InnerMost, we offer mental health care in Manhattan specifically for people navigating the emotional and psychological burden of chronic illness. Our integrated approach combines ketamine therapy, Spravato, and trauma-informed psychotherapy to address the depression, grief, and anxiety that chronic illness so often produces and sustains.
The Mental Health Burden of Chronic Illness
People with chronic illness are two to three times more likely to experience depression than the general population. Anxiety, grief, and a pervasive sense of loss, of the life one expected to have, of physical capacity, of certainty about the future, are nearly universal companions to serious long-term health conditions. Yet mental health care for this population remains an afterthought in most medical settings: something to address after the physical symptoms are managed, if it is addressed at all.
The relationship between chronic illness and mental health is not one-directional. Depression and anxiety worsen physical symptoms, reduce treatment adherence, increase pain perception, and diminish the quality of life that treatment is trying to protect. Addressing the psychological dimension of chronic illness is not a luxury. It is a meaningful part of how people survive it well.
At InnerMost, we take this seriously. We offer mental health care that is specifically attuned to the experience of living with a chronic condition: the grief that doesn’t resolve, the identity that has had to be rebuilt, the relationships strained by illness, and the particular kind of depression that comes not from a chemical imbalance in isolation but from circumstances that are genuinely, objectively hard.
- People with chronic illness are two to three times more likely to experience depression than the general population.
- Depression and anxiety worsen physical symptoms, reduce treatment adherence, and significantly diminish quality of life for people with chronic health conditions.
- The grief and identity loss that accompany chronic illness are real psychological injuries that deserve skilled clinical attention, not just reassurance.
Our Approach to Mental Health Care for Chronic Illness
Most therapists are not trained to work with the specific psychological landscape of chronic illness. The grief that keeps returning even when nothing new has happened. The anger at a body that feels like it has betrayed you. The exhaustion of being strong for other people. The medical trauma layered on top of the original condition. These require a particular kind of clinical attunement and skill, and they are at the center of what we offer.
Our integrated model brings together ketamine therapy, Spravato, and trauma-informed psychotherapy using IFS, AEDP, and Gestalt approaches. For people with chronic illness whose depression has not responded to conventional antidepressants, ketamine and Spravato offer a neurologically distinct path to relief, one that can produce meaningful results where SSRIs have not. And psychotherapy at InnerMost goes beyond symptom management to work with the deeper questions that chronic illness raises: who am I now, what does my life mean, how do I hold both the grief and the possibility of something good.
01
Why ketamine and Spravato for chronic illness-related depression?
Depression in the context of chronic illness is often treatment-resistant in practice, even when it does not technically meet the clinical definition, because conventional antidepressants have already been tried and found insufficient. Ketamine and Spravato work through an entirely different neurological mechanism, promoting rapid neuroplasticity and producing meaningful relief from depressive symptoms in ways that SSRIs cannot match on the same timescale. For someone whose energy and bandwidth are already significantly diminished by physical illness, waiting weeks for a medication to maybe work is not a neutral proposition. The speed of ketamine and Spravato matters here.
02
Why trauma-informed psychotherapy?
Chronic illness is frequently traumatic. Diagnosis, hospitalization, loss of function, medical procedures, the experience of being disbelieved or dismissed by providers: these are real traumatic events that leave their mark on the nervous system and the self. Our therapists are trained in IFS, AEDP, and Gestalt approaches that are specifically suited to working with this kind of accumulated medical and existential trauma, helping patients process what has happened, find coherence in their changed identity, and build a relationship with their life as it now is.
Who We Can Help
We work with adults in Manhattan and across New York City who are navigating the mental health dimensions of living with a chronic health condition. You do not need a specific diagnosis to work with us. What brings most of our patients here is a recognition that the psychological weight of their illness has become significant enough to need dedicated clinical attention, and that what they have tried so far has not been sufficient.
We work with people who are experiencing:
- Depression related to chronic illness, including treatment-resistant depression that has not responded to conventional antidepressants
- Anxiety, health anxiety, and fear of disease progression or the future
- Grief and mourning for the life, identity, and capacity that illness has changed or taken away
- Medical trauma from diagnosis, hospitalization, invasive treatment, or experiences of being dismissed or disbelieved by the medical system
- Relationship strain and isolation driven by the demands and limitations of chronic illness
- Existential distress and loss of meaning or purpose in the context of a changed life
Chronic illness rarely exists in a clean psychological silo. Many of the people we work with are also navigating depression, anxiety, trauma, and, in some cases, suicidal ideation that has emerged from the weight of their condition. Our integrated model is designed to hold that complexity with the care and depth it requires.

Treatments at InnerMost for Chronic Illness-Related Mental Health
Your InnerMost care team will determine the most appropriate treatment pathway during your initial consultation, taking into account your physical health, your mental health history, your current medications, and your goals.
For patients with chronic illness-related depression that has not responded to conventional antidepressants, ketamine therapy offers a neurologically distinct path to relief. InnerMost offers ketamine therapy through three formats, each with varying levels of psychotherapy integration, so care can be calibrated to your energy, capacity, and clinical needs. All formats include medical supervision and access to our licensed clinical team.
For patients whose chronic illness-related depression meets the criteria for treatment-resistant depression or MDD with acute suicidal ideation, Spravato may be an eligible and insurance-covered option. Spravato is covered by most major insurance plans and is administered in our private treatment rooms under clinical supervision, with monitoring and recovery time built into each session. Your InnerMost prescriber will assess your eligibility and review any interactions with your existing medications during your medical consultation.
Individual psychotherapy with an InnerMost therapist trained in IFS, AEDP, and Gestalt psychology is available as a standalone service or as the therapeutic backbone of a ketamine or Spravato treatment pathway. For people with chronic illness, this work addresses the grief, the medical trauma, the identity shifts, and the existential questions that illness raises, in a space where the full weight of the experience can be held with skill and genuine care.
What to Expect: Your Treatment Journey at InnerMost
We understand that people with chronic illness often have limited energy and complex medical situations. Our intake process is designed to be as low-friction as possible, and your care plan will always account for your physical health and capacity alongside your mental health needs.
01
Initial Consultation
You’ll begin with a 1-hour Medical Consultation and a 50-minute Therapy Consultation, typically both via telehealth. Your clinical team will take the time to understand your full picture: your illness history, your mental health, your current medications, and what you are hoping to find at InnerMost. The medical consultation will also assess any interactions between your existing medications or treatments and ketamine or Spravato, to ensure your care is safe and appropriate.
02
Preparation
Before your first dosing session, you’ll meet with an InnerMost therapist for a preparation session. This is an opportunity to set intentions, build rapport, and establish a therapeutic foundation before your first dosing session. For people with chronic illness, this often includes an exploration of what you are carrying, the grief, the exhaustion, the questions, so that the ketamine experience can be approached with clarity and intention.
03
Treatment Sessions
Treatment sessions take place in a private, calming room at our Manhattan center and are designed to be as comfortable as possible. Vital signs are monitored throughout, and your clinical team is attentive to your physical comfort at every stage. Sessions last 90 to 120 minutes including recovery time. If you have specific physical needs or limitations, please let your care team know during your consultation and we will accommodate them.
04
Integration
Within a few days of each dosing session, you’ll meet with your therapist for a dedicated integration session. This is where the therapeutic work deepens: processing what arose, working with grief and meaning, building new ways of relating to your illness and your life. Integration can also include practices like mindfulness, journaling, and somatic awareness, calibrated to your energy and physical capacity.
05
Ongoing Care
Mental health care in the context of chronic illness is rarely a short-term project. Your InnerMost team will work with you to build an ongoing care plan that is realistic, flexible, and genuinely responsive to how you are doing, both physically and psychologically. Your plan will be reviewed regularly and adjusted as your situation evolves.
Pricing & Insurance
We believe cost should not be an additional burden for people already navigating the financial weight of chronic illness. Here’s what you need to know:
Spravato®: covered by most insurance for eligible patients.
For patients whose depression meets the criteria for treatment-resistant depression or MDD with acute suicidal ideation, Spravato is covered by most major insurance plans including Aetna, Cigna, UnitedHealthcare, Empire BlueCross, Medicare, and Fidelis. Our team will work with you to verify your eligibility and navigate the process.
IV ketamine: not typically covered by insurance.
Most insurance plans do not cover IV ketamine directly. However, portions of your care, including the medical consultation and psychotherapy sessions, may be eligible for partial reimbursement. Our team can provide documentation to support insurance claims and help you explore FSA and HSA eligibility.
Psychotherapy: coverage varies.
Individual and integration therapy sessions may or may not be covered depending on your plan. Please reach out for current pricing and to discuss what may be available under your plan.
Frequently Asked Questions
Will my physical condition affect my eligibility for ketamine or Spravato?
It may, depending on your specific diagnosis, current medications, and medical history. Your InnerMost prescriber will conduct a thorough medical review during your initial consultation to assess safety and eligibility. Many people with complex chronic health conditions are able to receive ketamine or Spravato safely, but this is always assessed on an individual basis. We will be transparent with you about what we find and work with your other providers as needed.
Can InnerMost coordinate with my other medical providers?
Yes. We are experienced in coordinating with outside providers and understand that people with chronic illness often have complex, multi-provider care situations. Please let us know during your initial consultation who else is involved in your care, and we will work collaboratively to ensure your treatment at InnerMost is coherent with everything else you are managing.
I don't have much energy. Is treatment at InnerMost realistic for me?
We hear this often, and we take it seriously. Initial consultations are via telehealth, which removes the burden of travel. Treatment sessions at our center are designed to be restful and low-demand on your part. Integration sessions can be kept shorter or scheduled flexibly depending on your capacity. We will build a care plan around your actual energy and limitations, not around what would be ideal in the abstract.
Is psychotherapy alone an option if I'm not ready for ketamine or Spravato?
Absolutely. Individual psychotherapy with an InnerMost therapist is available as a standalone service, and for many people with chronic illness it is the right starting point. Our therapists are trained in modalities that are specifically well-suited to the grief, trauma, and existential dimensions of chronic illness. You do not need to pursue ketamine or Spravato to receive meaningful care at InnerMost.
How is InnerMost different from seeing a general therapist for chronic illness support?
Most therapists are not specifically trained to work with the psychological landscape of chronic illness: the particular quality of grief that keeps returning, the medical trauma, the identity disruption, or the existential weight of a life that has changed in fundamental ways. Our therapists bring training in IFS, AEDP, and Gestalt psychology, as well as a clinical context that includes ketamine and Spravato as potential components of care, meaning the therapeutic work is informed by and connected to everything else InnerMost offers.
How do I get to InnerMost from around Manhattan?
