Substance Use Disorder Treatment in Manhattan, NYC

Substance use disorder is not a moral failing. It is a complex condition rooted in neurobiology, trauma, and the very human need to find relief from pain that has felt unmanageable. At InnerMost, we offer substance use disorder treatment in Manhattan that begins from this understanding, one that is compassionate, non-judgmental, and clinically rigorous. Our integrated approach combines ketamine-assisted psychotherapy, trauma-informed psychotherapy, and psychedelic integration support to address the psychological and neurological roots of addiction, not just its surface behaviors. We work alongside people ready to do the deeper work of lasting change.

Understanding Substance Use Disorder

Substance use disorder (SUD) develops when the relationship between a person and a substance, whether alcohol, opioids, stimulants, cannabis, or others, shifts from use to compulsive dependence that causes significant harm. It is not simply a habit or a lack of willpower. It is a condition that reshapes the brain’s reward and stress systems over time, making the substance feel necessary in ways that override judgment, relationships, and self-care.

At the neurological level, substances of misuse flood the brain’s reward circuitry with dopamine, creating powerful reinforcement signals that conventional motivation struggles to compete with. Over time, the brain adapts by reducing its baseline dopamine activity, leaving a person feeling flat, anhedonic, and unable to find pleasure in things that once mattered. This is not weakness. It is neuroadaptation, and it is one of the reasons why willpower alone is rarely sufficient for lasting recovery.

Substance use disorder almost always has a psychological underpinning. Trauma, depression, anxiety, chronic pain, and relational suffering are among the most common drivers of addictive behavior. The substance becomes a solution, however destructive, to a pain that has not found another outlet. At InnerMost, we treat the whole person, addressing not just the substance use itself but the underlying conditions that have sustained it.

  • Substance use disorder affects an estimated 46 million Americans, yet fewer than 20 percent receive any form of treatment in a given year.
  • SUD almost always co-occurs with depression, anxiety, trauma, or chronic pain, and treating each in isolation leaves the full picture unaddressed.
  • Ketamine-assisted psychotherapy is one of the most promising emerging treatments for addiction, working at the neurological level to disrupt entrenched patterns and open a window for genuine therapeutic change.

A Different Approach to Substance Use Disorder Treatment

Conventional addiction treatment in Manhattan typically focuses on detoxification, medication-assisted treatment, 12-step programs, and behavioral therapy. These approaches have helped many people, and we respect the role they play. But for a significant number of individuals, they address the behavior without reaching the root, and relapse remains the norm rather than the exception.

At InnerMost, we offer something complementary and, for many people, transformative: an approach that works at the neurological and psychological depth that addiction requires. Ketamine-assisted psychotherapy disrupts the entrenched neural patterns that sustain addictive behavior, creates a non-ordinary state in which the psychological roots of addiction can be approached with new spaciousness, and promotes the neuroplasticity that makes genuine behavioral change possible. Trauma-informed psychotherapy addresses the pain, relational wounds, and self-narrative that have driven the substance use. And integration work ensures that what shifts during treatment is woven into lasting change.

We are not a detox facility, and we do not offer medication-assisted treatment for withdrawal. What we offer is the deeper psychological and neurological work that helps people understand why they used, address what drove the use, and build new internal conditions that make recovery sustainable.

01

Why ketamine-assisted psychotherapy for addiction?

Addiction encodes itself in the brain’s habit and reward systems with remarkable tenacity. Conventional therapy, delivered in an ordinary state of consciousness, often struggles to access and shift these deeply encoded patterns. Ketamine’s action on the glutamate system temporarily disrupts this encoding, creating a window of neuroplasticity during which new associations, new responses, and new ways of relating to craving and discomfort become genuinely accessible. Research into ketamine for alcohol use disorder and other addictions is still developing, but the early evidence is promising, and the clinical rationale is compelling.

02

Why trauma-informed psychotherapy?

Substance use disorder is rarely a standalone condition. The majority of people with SUD have a significant trauma history, and the substance has often served, at least in part, as a way of managing traumatic material that has had nowhere else to go. At InnerMost, our therapists are trained in IFS, AEDP, and Gestalt psychology, modalities that are particularly effective for trauma and the relational wounds that often underlie addiction. Addressing this material, in a safe, skilled therapeutic relationship, is essential to recovery that lasts.

03

Why psychedelic integration?

Many people navigating substance use disorder have had psychedelic experiences, whether intentional or incidental, that have produced insights about their relationship with substances that they haven’t known how to work with. Others have undergone psychedelic-assisted experiences, legally abroad or in underground settings, as part of their recovery journey, without the aftercare support to integrate what arose. InnerMost’s harm reduction and integration services are available to anyone in this position, providing the skilled therapeutic support needed to make those experiences therapeutically meaningful.

Substance Use Disorder Treatments at InnerMost

We offer multiple evidence-based treatment options relevant to substance use disorder. Your InnerMost care team will determine the most appropriate pathway during your initial consultation, taking into account your substance use history, co-occurring conditions, and treatment goals.

01

Ketamine-Assisted Psychotherapy (KAP)

KAP is the primary treatment modality at InnerMost for people with substance use disorder. In KAP, a licensed therapist is present throughout the ketamine dosing session, helping you navigate the non-ordinary state and work with material as it arises in real time. For people with addiction, this format is particularly well-suited: the psychological roots of substance use, including underlying trauma, shame, and unmet need, often surface during the experience in ways that become accessible to therapeutic work for the first time. KAP sessions are followed by dedicated integration sessions in the days afterward, where this work is continued and consolidated.

02

Ketamine Infusion With Integration (KIWI)

For patients whose presentation calls for IV ketamine but who are not yet ready for a therapist to be present during the dosing session, KIWI provides intravenous infusions with built-in preparation and integration sessions with an InnerMost therapist. This format allows patients to experience the neuroplasticity benefits of ketamine while receiving robust therapeutic support before and after each session.

03

Trauma-Informed Psychotherapy

For patients who are not yet ready for ketamine, or who want to begin with a strong therapeutic foundation, InnerMost offers individual psychotherapy with licensed therapists trained in IFS, AEDP, and Gestalt psychology. These approaches are among the most effective available for the trauma, shame, and relational patterns that commonly underlie substance use disorder. Psychotherapy is also the essential ongoing support for patients engaged in ketamine treatment, where integration work between sessions is central to lasting recovery.

04

Harm Reduction & Integration Support

For individuals navigating substance use disorder who have had psychedelic experiences, in legal contexts, on retreat abroad, or in underground settings, and who need skilled support in integrating those experiences, InnerMost’s harm reduction and integration services are available. We meet people where they are, without judgment, and help them make therapeutic use of what they have experienced.

Who We Can Help

We work with adults in Manhattan and across New York City who are navigating substance use disorder and the conditions that most commonly accompany it. InnerMost is not a detox facility or a residential treatment program. We are an outpatient clinic offering the deeper psychological and neurological work that supports recovery over the long term.

We work with people who are:

  • Seeking to understand and address the psychological roots of their substance use alongside or following primary addiction treatment
  • Managing alcohol use disorder and looking for a more integrated, therapeutically grounded approach to recovery
  • Navigating substance use that co-occurs with depression, anxiety, PTSD, or chronic pain, and who need care that addresses the full picture
  • In sustained recovery but struggling with the underlying depression, trauma, or emotional dysregulation that has driven use in the past
  • Looking for a non-judgmental clinical environment that honors their autonomy and meets them where they are
  • Interested in the role ketamine-assisted psychotherapy may play in supporting their recovery journey

We do not require abstinence as a precondition for care. We meet people where they are and work collaboratively toward goals they define. Recovery looks different for different people, and our model is built around that reality.

How InnerMost's Approach to Substance Use Disorder Works

Our integrated model addresses substance use disorder on multiple levels. Here is what the research and our clinical experience show:

Disrupting entrenched neural patterns.

Addiction encodes itself in the brain’s habit circuitry with a persistence that conventional therapy delivered in ordinary states of consciousness often struggles to match. Ketamine’s action on the glutamate system temporarily disrupts this encoding, creating a window of neuroplasticity during which new associations and new responses to craving and distress become genuinely accessible. For people whose relationship with a substance has felt compulsive despite their own contrary intentions, this neurological interruption can be one of the most meaningful things they have ever experienced in treatment.

Accessing and processing the psychological roots.

The non-ordinary state produced by ketamine allows people to approach the psychological material underlying their substance use, the pain, the shame, the unmet need, the traumatic history, with a degree of spaciousness and self-compassion that can be genuinely difficult to access in an ordinary state. Material that has been defended against for years sometimes becomes approachable for the first time. In KAP, a trained InnerMost therapist is present to help you navigate this with skill and care.

Addressing co-occurring depression and trauma.

Substance use disorder and depression co-occur in roughly a third to half of all cases. Trauma histories are even more prevalent. At InnerMost, our integrated model addresses these conditions simultaneously rather than sequentially, using the same therapeutic framework, the same care team, and the same integration work to reduce the burden of each. When the depression lifts and the traumatic material is processed, the compulsion to use often loosens alongside it.

Rebuilding relationship with self.

One of the most corrosive aspects of long-term substance use disorder is what it does to self-perception: the accumulated shame, the fractured sense of identity, the conviction that one is fundamentally broken or beyond help. IFS and AEDP are particularly powerful tools for working with this material, helping patients develop a more compassionate, coherent relationship with themselves, one in which recovery becomes something to move toward rather than something to perform.

Integration as the engine of lasting change.

The insights and shifts that occur during a ketamine session are not self-implementing. Integration therapy, the dedicated sessions with your therapist in the days following each dosing session, is where those shifts are processed, consolidated, and woven into daily life. For people with substance use disorder, this integration work often includes exploring the role the substance has played, developing new coping strategies, and building new internal conditions that make recovery sustainable rather than effortful.

What to Expect: Your Treatment Journey at InnerMost

Every person who comes to InnerMost for substance use disorder treatment has a different history, a different relationship with substances, and different goals. Here is how the process typically unfolds:

01

Initial Consultation

You’ll begin with a 1-hour Medical Consultation and a 50-minute Therapy Consultation, typically both via telehealth. These sessions give your clinical team a thorough picture of your substance use history, your co-occurring conditions, your previous treatment experiences, and your current goals. There is no judgment here. Our goal is to understand your full picture and work with you to determine the most appropriate pathway forward.

02

Medical Review and Safety Assessment

Before any ketamine treatment begins, your InnerMost prescriber will conduct a thorough review of your medical history, current medications, and substance use to ensure ketamine is safe and appropriate for you. Certain substances and medications require careful management in relation to ketamine, and your clinical team will guide you through this honestly and transparently.

03

Preparation

Preparation sessions with your InnerMost therapist lay the groundwork for your dosing sessions: setting intentions, building the therapeutic relationship, developing a framework for working with difficult material, and establishing a clear sense of what you hope to understand or shift. For people with substance use disorder, this preparatory work often includes an exploration of the psychological function the substance has served and what you are ready to look at.

04

Treatment Sessions

KAP sessions take place in a private, calming treatment room with your therapist present throughout the dosing experience. KIWI sessions are administered with your therapist available before and after. In both cases, vital signs are monitored throughout and the clinical team ensures your safety and comfort. Sessions last 90 to 120 minutes, followed by recovery time in our private recovery lounge.

05

Integration

Integration sessions, held within a few days of each dosing session, are the heart of the work at InnerMost. In these dedicated therapy sessions, you and your therapist process what arose during the experience: insights about the role substances have played, emotions that surfaced, shifts in self-perception, and new perspectives on your history and your path forward. Integration can also include practices like journaling, mindfulness, somatic awareness, and other tools for building new patterns between sessions.

06

Ongoing Care

KAP sessions at InnerMost are generally scheduled one to two times per month, with ongoing psychotherapy between sessions strongly encouraged. Recovery from substance use disorder is a long-term process, and your InnerMost team will work with you to build a care plan that reflects that reality. Your plan will be reviewed regularly and adjusted as your process and goals evolve.

A Space Designed for Your Healing

For many people navigating substance use disorder, clinical settings have been places of judgment, protocol, and shame. At InnerMost, our Manhattan center at 137 East 25th Street, 11th Floor, in the Gramercy neighborhood was designed to be something different: a space where the nervous system can settle, where safety is felt rather than merely stated, and where the work of deep inner change can unfold with the support it requires.

From the calming materials and private treatment rooms to the recovery lounge where you rest after a session, every detail was chosen with your comfort and inner experience in mind. You will be met here as a whole person, not a diagnosis.

Our center is easily accessible from Midtown, Flatiron, Gramercy, Chelsea, and the East Village. Telehealth consultations are also available for New York residents.
