Understanding Suicidal Ideation
Suicidal ideation — thoughts about ending one’s life — exists on a spectrum. For some people it is passive: a recurring wish not to be here, a sense that others would be better off without them, or a fantasy of escape from pain that feels unbearable and permanent. For others it is more active: intrusive thoughts about methods, plans, or timing. Both are serious. Both deserve clinical attention, not judgment.
Suicidal ideation is most commonly a symptom of an underlying condition, particularly major depressive disorder (MDD), treatment-resistant depression (TRD), PTSD, or a combination of these. It is the mind’s response to pain that has exceeded its capacity to cope, not a character flaw, not a moral failure, and not a fixed or inevitable state. With the right treatment, it can be addressed, and the conditions that give rise to it can change.
For many years, the clinical options for people experiencing suicidal ideation as part of MDD were limited: hospitalization, medication adjustment, and supportive therapy. Ketamine and Spravato have changed that picture meaningfully. Both have demonstrated the ability to reduce suicidal ideation rapidly, often within hours, offering a degree of acute relief that conventional antidepressants simply cannot provide on that timescale.
- Spravato (esketamine) is the only FDA-approved treatment specifically indicated for major depressive disorder with acute suicidal ideation or behavior.
- Ketamine has demonstrated rapid reduction in suicidal ideation in clinical research, often within hours of administration.
- Suicidal ideation is a symptom of treatable conditions, not an immutable state, and effective, evidence-based care exists.
Our Approach to Treating Suicidal Ideation
At InnerMost, we treat suicidal ideation as a clinical symptom embedded in a larger picture: the depression, trauma, or accumulated burden that has brought a person to this point. Our goal is not only to reduce the acuity of suicidal thoughts in the short term but to address the underlying conditions that sustain them, with the full depth of integrated clinical care.
Our approach combines three elements: the rapid neurological action of Spravato or ketamine to provide acute relief; trauma-informed psychotherapy to address the psychological and relational roots of the suffering; and integration work to help what shifts during treatment become lasting change. We do not treat suicidal ideation as a crisis to be managed and discharged. We treat it as part of a person’s story, one that deserves to be understood and worked with over time.
01
Spravato®: the only FDA-approved option for MDD with suicidal ideation.
Spravato is specifically FDA-approved for major depressive disorder with acute suicidal ideation or behavior, making it the most rigorously validated pharmacological option for this presentation. It works through the glutamate system, producing rapid neuroplasticity and a reduction in both depressive symptoms and suicidal ideation that conventional antidepressants cannot match on the same timescale. It is covered by most major insurance plans, including Aetna, Cigna, UnitedHealthcare, Empire BlueCross, Medicare, and Fidelis, making it one of the most accessible advanced treatments available in Manhattan.
02
Ketamine therapy: rapid relief with therapeutic depth.
For patients who are not eligible for Spravato or whose clinical presentation calls for a different approach, ketamine infusion therapy and ketamine-assisted psychotherapy offer meaningful alternatives. Clinical research has consistently demonstrated ketamine’s capacity to reduce suicidal ideation rapidly, often within hours of the first session, and its promotion of neuroplasticity creates a window during which therapeutic work can produce unusually deep and lasting change.
03
Psychotherapy: addressing the roots, not just the symptoms.
Suicidal ideation does not arise in a vacuum. It is the expression of suffering that has accumulated over time, shaped by depression, trauma, loss, relational pain, or a combination of these. At InnerMost, every clinical pathway includes access to licensed therapists trained in IFS, AEDP, and Gestalt psychology, approaches specifically suited to working with the depth of pain that underlies suicidal ideation. This therapeutic work is the foundation of lasting recovery, not an adjunct to it.
Treatments for Suicidal Ideation at InnerMost
Your InnerMost care team will determine the most appropriate treatment pathway during your initial consultation, taking into account your clinical history, current symptom severity, and overall presentation.
01
Spravato® (Esketamine)
Spravato is the primary treatment we recommend for patients whose suicidal ideation meets the criteria for MDD with acute suicidal ideation or behavior. It is self-administered as a nasal spray under direct clinical supervision and has demonstrated significant, rapid reductions in both depressive symptoms and suicidal thoughts in clinical trials. At InnerMost, Spravato is offered through three pathways, each with varying levels of psychotherapy integration, so your care can be calibrated to the severity and complexity of your presentation. Spravato is covered by most major insurance plans.
02
Ketamine Therapy
For patients who do not meet the specific FDA criteria for Spravato, or whose clinical presentation is better suited to intravenous ketamine, InnerMost offers ketamine infusion therapy and ketamine-assisted psychotherapy. Both have demonstrated efficacy in reducing suicidal ideation rapidly, and both are delivered within a fully integrated therapeutic framework that includes preparation and integration sessions with a licensed therapist.
03
Psychotherapy
All patients receiving Spravato or ketamine at InnerMost have access to licensed psychotherapists trained in IFS, AEDP, and Gestalt psychology. For patients who are stabilizing and not yet ready for a clinical treatment, or who want to begin with a strong therapeutic foundation, individual psychotherapy is also available as a standalone starting point. Our therapists are experienced in working with suicidal ideation, depression, and the complex presentations that often accompany them.
Who We Can Help
InnerMost provides ongoing clinical care for adults in Manhattan and across New York City who are experiencing suicidal ideation as part of a diagnosable mental health condition. We are not an emergency or crisis service. If you are in immediate danger, please call or text 988 or go to your nearest emergency room.
We work with adults who are:
- Experiencing passive or active suicidal ideation as part of major depressive disorder (MDD) or treatment-resistant depression (TRD)
- Managing suicidal thoughts that co-occur with PTSD, trauma, anxiety, or substance use disorder
- Stable enough to engage in outpatient care but looking for a more effective, integrated treatment approach than what they have tried before
- Currently on medication or in therapy but not finding adequate relief from suicidal ideation
- Looking for a clinical environment where they can speak openly about suicidal thoughts without fear of automatic hospitalization or judgment
We understand that one of the reasons people do not seek help for suicidal ideation is fear: fear of being hospitalized, fear of being labeled, fear that honesty will result in a loss of control over their own care. At InnerMost, we take a collaborative, patient-centered approach. Our goal is to provide the most effective outpatient care possible and to support your autonomy throughout the process. We will always prioritize your safety, and we will always be honest with you about when a higher level of care may be needed.

How InnerMost's Approach Works
Our integrated model addresses suicidal ideation on multiple levels simultaneously. Here is what the research and our clinical experience show:
Rapid reduction in suicidal ideation.
Both Spravato and ketamine have demonstrated the ability to reduce suicidal ideation significantly and rapidly, in some clinical trials within hours of a single session. For people whose suicidal thoughts have been present for weeks, months, or years, this speed is not a minor detail. It is a meaningful change in the immediate risk landscape, one that creates space for the deeper therapeutic work to begin.
Targeting the underlying depression.
Suicidal ideation in the context of MDD is, in part, a symptom of a brain that has been in a depressive state long enough to lose perspective on its own capacity for change. Spravato and ketamine directly interrupt this state at the neurological level, reducing depressive symptoms and restoring a degree of cognitive flexibility that suicidal thinking tends to foreclose. When the depression lifts, even partially, the suicidal ideation typically follows.
Neuroplasticity and new possibilities.
One of the cognitive signatures of suicidal ideation is tunnel vision: the conviction that things will not and cannot change, that the current state is permanent, and that there are no other options. Ketamine and Spravato promote neuroplasticity, the formation of new neural connections that can physically expand the range of what feels possible. This is not simply a mood effect. It is a structural shift in the brain’s capacity to generate alternatives.
Therapeutic work on the roots of suffering.
Suicidal ideation is almost always the surface expression of something deeper: accumulated pain, relational loss, unprocessed trauma, or a self-narrative built around worthlessness and hopelessness. At InnerMost, our therapists use IFS, AEDP, and Gestalt approaches to work with this material directly, helping patients develop a more compassionate relationship with the parts of themselves that are in pain and building new internal conditions that make suicidal ideation less necessary as a coping response.
A safe clinical environment.
For many people with suicidal ideation, the clinical encounter itself is fraught: a history of being hospitalized against their will, of having their words used against them, of feeling more like a liability than a person. At InnerMost, we approach every patient with dignity and transparency. We are not here to manage risk at the expense of the therapeutic relationship. We are here to provide the most effective care possible, in a space where you feel safe enough to be honest.
What to Expect: Your Treatment Journey at InnerMost
Beginning treatment for suicidal ideation takes courage. Here is how the process typically unfolds at InnerMost, and what you can expect at each stage:
01
Initial Consultation
You’ll begin with a 1-hour Medical Consultation and a 50-minute Therapy Consultation, typically both via telehealth. These sessions give your clinical team a thorough and compassionate picture of what you are experiencing, your history with depression or other conditions, what treatments you have tried, and what is happening now. Safety is assessed collaboratively and respectfully, not as a gatekeeping exercise but as a foundation for understanding how best to support you.
02
Determining Your Pathway
Based on your consultations, your clinical team will determine whether Spravato, ketamine therapy, or psychotherapy alone is the most appropriate starting point. For patients meeting the criteria for MDD with acute suicidal ideation, Spravato is often the first recommendation given its FDA approval and insurance coverage. The decision will always be made collaboratively, with your needs and preferences fully considered.
03
Preparation
Before your first dosing session, you’ll meet with a licensed InnerMost therapist for a preparation session. This is an opportunity to build safety and rapport, set intentions for the experience, and develop a plan for navigating difficult material if it arises. For patients working with suicidal ideation, this stage is particularly important, and we take the time needed to ensure you feel genuinely ready.
04
Treatment Sessions
Spravato sessions take place in a private, calming treatment room and last approximately 120 minutes including monitoring and recovery time. Ketamine infusion sessions last 90 to 120 minutes. Vital signs are monitored throughout. Depending on your treatment pathway, an InnerMost therapist may be present before and after administration. You are never left alone during a session.
05
Integration
Within a few days of each dosing session, you’ll meet with your therapist for a dedicated integration session. This is where the neuroplasticity window is met with therapeutic depth: processing what arose, working with any shifts in perspective, and continuing the work on the underlying pain that has driven the suicidal ideation. Integration is not optional in our model. It is where the treatment becomes lasting change.
05
Ongoing Care and Safety Planning
Your InnerMost team will maintain close contact throughout your treatment course and check in regularly on how things are progressing. Safety planning is a collaborative, ongoing process, not a one-time form. If at any point your clinical team believes a higher level of care is needed, they will discuss this with you directly and support you in accessing it.
A Space Designed for Your Healing
For people carrying the weight of suicidal ideation, walking into a clinical space can feel daunting. At InnerMost, our Manhattan center at 137 East 25th Street, 11th Floor, in the Nomad neighborhood was designed to feel nothing like the clinical environments that can make that weight heavier. Calming materials, private treatment rooms, and an atmosphere of safety and containment: every detail was chosen with the nervous system in mind.
You will not be met with clipboards, cold lighting, or clinical distance. You will be met by people who understand the depth of what you are carrying and who are here to help you carry it differently.
Our center is easily accessible from Midtown, Flatiron, Gramercy, Chelsea, and the East Village. Telehealth consultations are also available for New York residents.

Frequently Asked Questions About Suicidal Ideation Treatment in Manhattan
Will I be hospitalized if I talk about suicidal thoughts at InnerMost?
Not automatically, and not without a transparent conversation with you first. At InnerMost, we take a collaborative approach to safety. We will always be honest with you about our clinical assessment, and if we believe a higher level of care is needed, we will discuss that with you directly and support you in accessing it. Our goal is to provide the most effective outpatient care possible, not to manage risk at the expense of the therapeutic relationship. Honesty about your experience will not be used against you.
Is Spravato specifically approved for suicidal ideation?
Yes. Spravato (esketamine) is FDA-approved for major depressive disorder with acute suicidal ideation or behavior, making it the only pharmacological treatment specifically indicated for this presentation. Clinical trials demonstrated significant and rapid reductions in both depressive symptoms and suicidal ideation, often within hours. It is covered by most major insurance plans for eligible patients.
How quickly can treatment reduce suicidal ideation?
Both Spravato and ketamine have demonstrated the ability to reduce suicidal ideation significantly within hours to days of a first session in clinical research. This is one of the most clinically meaningful distinctions between these treatments and conventional antidepressants, which can take weeks to produce any effect. Individual responses vary, and your InnerMost team will monitor your progress closely and adjust your plan as needed.
Can I receive treatment at InnerMost if I am currently seeing another psychiatrist or therapist?
Yes. Many of our patients come to InnerMost as an augmentation of existing care. We are experienced in coordinating with outside providers and will work collaboratively with your current team to ensure your care is coherent and continuous. Please let us know during your initial consultation who else is involved in your care.
What if I am in crisis right now?
If you are in immediate danger, please call or text 988 (Suicide & Crisis Lifeline) or go to your nearest emergency room. InnerMost provides ongoing clinical care for people experiencing suicidal ideation as part of a mental health condition, but we are not an emergency or crisis service. Once you are safe and stable, we are here to help you build toward lasting recovery.
What makes InnerMost different from other providers treating suicidal ideation in NYC?
We offer Spravato, the only FDA-approved treatment for MDD with acute suicidal ideation, within a fully integrated therapeutic framework that includes preparation and integration sessions with licensed psychotherapists trained in IFS, AEDP, and Gestalt psychology. We do not administer medication and send patients home. We treat suicidal ideation as a symptom embedded in a larger story, one that requires depth, continuity, and genuine clinical partnership to address.
Does InnerMost accept insurance for suicidal ideation treatment?
Spravato is covered by most major insurance plans for eligible patients with MDD and acute suicidal ideation. IV ketamine is not typically covered by insurance, though some elements of care may qualify for reimbursement. Psychotherapy coverage varies by plan.
